Elderly patient arrives via EMS in severe respiratory distress. He becomes increasingly distressed and agitated, requiring intubation. The patient develops a tension pneumothorax and may arrest.

Severe COPDE w/ Pneumothorax

An elderly patient has just arrived via EMS in severe respiratory distress. He has a history of COPD and is on home O2. He's had 3 days of worsening cough, sputum, and dyspnea. He becomes increasingly distressed and agitated, requiring intubation. Learners may consider non-invasive positive pressure ventilation (NIPPV), delayed sequence intubation (DSI), but the patient will ultimately require intubation. Shortly after intubation, the patient becomes very difficult to bag and then becomes hypotensive. If not identified, the patient arrests from a tension pneumothorax.
