ØRECLESS: Shoulder Dystocia in a Rural Family Practice.

It’s Canada Day weekend and you’re the physician on-call for obstetrics at a rural 1A maternity site (no local cesarean section capabilities). The hospital is staffed by one other physician who is on-call to the ER, two registered nurses (RNs), and a registered practical nurse (RPN). All the other doctors and nurses are out of the community enjoying the wild. The other physician, an RN, and the RPN are currently in the ER dealing with an acute trauma when the remaining RN calls the obstetrics provider into the delivery room.

You’re rounding on patients at the hospital when a 34 y/o woman presents to the ED at 40 weeks gestation with approximately four hours of labour pain. On examination, the fetus will be in a cephalic presentation and will develop shoulder dystocia. The learner must deliver the fetus from shoulder dystocia using various maneuvers with the assistance of one novice RN and the woman’s support person. The learner will practice auscultation and manual maneuvers for the management of shoulder dystocia.
